Global Concepts Charter K-8 School

Students who take medication(s) during school will need to have a new medication order form filled out from the physician every school year. Please remember:

• A physician must indicate on the medication order form if your child can self-carry and self-administer.
• A parent/guardian signature on the form is also required.
• A parent/guardian needs to bring in the medication to the Nurse, please do not send it with your student.
